Adding patient forms to the website is a good way to save your patient’s time and improve patient satisfaction. To add your forms, first convert any Word Document into a PDF with the 'Save as...' option in Word. It helps to understand Inserting an Image and Hyperlinking to properly implement a patient form.
Upload the PDF using the ‘Upload a new asset’ button on the Admin View Assets Page. In this example we will also upload a forms icon and link that image to the PDF form.
Tip: Providing an Optional Filename will keep your files organized. For example, “Einstein Blog User Guide pdf”.
We found this icon in Google Images. Add the form icon to the page and section you want your forms available from.
Tip: Introduce your online resources with a sentence or two.
Next, access the image properties by selecting the image in the Content Editor and then clicking the ‘Image’ button. Then select the ‘Link’ tab and click the ‘Asset Manager’ button. Find your form in the list of assets and ‘Select’ it. Now choose whether your form will open up in the same window or a new window and click ‘OK’.
You can preview the addition to your content, then publish to the live web.
Tip: It is better to link to a new window for your forms so when the patient closes the form window, he or she is back on your site.
